VACCINATION EXEMPTIONS

The exemption form here, is for residents of Pennsylvania only.  As far as I know, all states allow a medical exemption. Some allow philosophical and/or religious exemptions. However, my research tells me that the American Medical Association is actively promoting the abolishment of religious and philosophical exemptions.
